Transition Team - Liverpool
Liverpool City Council Adult Services are currently working with our colleagues in Children's Services, Education, Health, Schools and Colleges to develop a revised pathway to adulthood for young people who are in transition.
We are seeking to deliver, with our partnership agencies, a co-ordinated multi agency support service for young people to achieve good life outcomes.
We have a duty to carry out a transition assessment for a young person and / or their carer, in order to help them plan if they are likely to have support needs once they turn 18.
If you are a young person or young carer and you are likely to have needs when you become 18, we would like to know about you and offer you an assessment of your needs. This is called a "Supported Self Assessment" under the Care Act 2014. If you would like to request an assessment and you do not have a children's Social Worker, please make contact with Careline on 0151 233 3800.
If you are already known to Children's Services then your Social Worker will make a referral.
We will enable, assist and encourage you to take an active part in your Supported Self Assessment.
If you are a carer for a young person who is likely to have support needs when they have turned 18 then we would like to hear from you, in order for a Carer Assessment to be offered to you. Please contact Careline on 0151 233 3800.
We are holding information meetings at Schools to talk about the transitionsprocess, share information and find out about your carers and what you think is important.
