PSS - Community Support

A community based support service for people aged 16+ with learning disabilities, mental health needs, physical or sensory impairment.

This service is to enable people to make use of ordinary  facilities and opportunities. It is part of PSS Home and Communities services.

Community Support provides support to individuals to enable them to access employment, education and increase skills and confidence. We strive to empower people to live as independently as possible and reach their true potential.

We work with individuals and their families to agree their own outcomes and put together a bespoke support plan. Our support workers are carefully selected and trained on an ongoing basis. each of them are either working towards or are fully qulaified with a QCF Level 2 in Health & Social Care. The team all have experience of supporting people in the community and have a flexible approach in how they work.

Community Support operates seven days a week between 8am and 10pm (sleepovers can be arranged if required).

Referrals are welcome from individuals themselves, family members or professionals.
